These are three-quarter-ton and one-ton trucks, sold new as the Dodge Ram 2500 and 3500, with a dually option on the 3500. The High Output 5.9L Cummins is rated 305 hp and 555 lb-ft. Cummins 2500 and 3500 trucks use the American Axle 11.5 in rear axle, with 3.73 or 4.10 gearing from the factory; the 4.10 is the heavy-tow ratio. High Output trucks came with the NV5600 six-speed manual or the 48RE four-speed automatic. On the 48RE, tow/haul mode engages converter lockup and keeps the transmission from upshifting into overdrive, and that is the mode to use with a trailer behind you.
What fails and why
Three problems show up on these trucks once a heavy trailer goes on the hitch:
- 48RE heat. Towing in overdrive drives fluid temperature up. Past 260F, the transmission forces lockup and a 4-3 downshift to protect itself. Watch for those protective downshifts, dark fluid and a lockup that slips. Converter and band wear speed up with trailer weight, and transmission builders advise not letting the factory intermediate band go past 120,000-150,000 miles.
- Rear sag. Tongue or pin weight squats the rear leaf packs. The truck sits nose-high, steering suffers and the headlights aim high.
- EGT on long grades. With a trailer on a long climb, the stock turbo and 3.5 in exhaust let exhaust gas temperature sit above 1,250F. The 2003-2004 engine's maximum is 1,450F.
Stock replacement vs. upgrade
A towing setup is only as strong as its lightest-rated piece, whether that is the ball, the ball mount or the receiver, so hitch hardware is the first thing to get right. Past that, Diesel Hub's towing list for trucks like this one includes air bags, a deep transmission pan and cooler, a pyrometer and gauges, and synthetic fluids. For trucks that tow hard, transmission builders also recommend a built 48RE converter and an upgraded intermediate band. Air bags are the item Diesel Hub calls a must-have for trucks that run at or close to their GCWR. An adjustable ball mount is an easy upgrade too: it lets you set the trailer level instead of living with a mount that is almost right.

Fitment notes
Bed-mounted hitches need the most checking. This generation has no factory puck system, so gooseneck and 5th wheel hardware bolts to the frame through the bed floor; match the kit to your bed length and to single or dual rear wheels. The 3500 dually and the single-rear-wheel trucks share the 11.5 in rear axle but differ in axle rating and in fender and hitch clearance. Short beds need a 5th wheel slider or careful ball placement to keep the trailer off the cab. Cab choices for these years are Regular Cab and Quad Cab; the Mega Cab did not arrive until 2006. Several receivers here are listed across 2003-2018 or 2003-2009, so read each product's years and verify by VIN.
Frequently asked questions
How much can my 2003-2004 Ram 2500 or 3500 tow?
We don't publish a number. Factory tow ratings for these trucks depend on cab, axle ratio and transmission, and we could not confirm them from a primary source. Use the GVWR and GCWR on your door-jamb certification label: GVWR is the most the truck can weigh with everything in it, and GCWR is the most the truck, trailer and all cargo can weigh together.
Why does my transmission downshift by itself when I tow?
That is the 48RE protecting itself. When fluid temperature passes 260F in overdrive, it forces lockup and drops from fourth to third. Tow in tow/haul mode, which locks the converter and blocks overdrive upshifts, and treat repeated downshifts or dark fluid as a sign the transmission needs a look.
Do I need air bags to tow with this truck?
If your trailer puts real weight on the hitch or in the bed, they help. Rear sag on a loaded 2003-2004 leaves the truck nose-high, with the headlights aimed up and steering that suffers. Diesel Hub calls air bags a must-have for trucks running at or near GCWR. They level the truck; they do not raise the ratings on the label.
Gooseneck or 5th wheel?
Your trailer's coupler usually decides. A gooseneck ball sits in the bed above the rear axle, is fixed in place and leaves the bed open when you're not towing. A 5th wheel uses a bed-mounted plate and pin, adjusts fore and aft, and takes up bed space. On a short bed, either one needs careful placement to avoid cab contact.
What exhaust temperature is too hot when towing?
Keep sustained EGT under about 1,250-1,300F, measured before the turbo. The 2003-2004 5.9L tops out at 1,450F, lower than later 5.9L trucks. A pyrometer is how you see it, and Diesel Hub lists one with gauges among its core towing mods.
