Mid-1998, the 12-valve and its mechanical pump gave way to the 24-valve ISB and an electronically controlled Bosch VP44 rotary pump. On these trucks an electric lift pump bolted to the side of the block, behind the filter, pulls fuel from the tank, pushes it through the filter and feeds the VP44. The VP44 times and meters fuel to six mechanical injectors through individual high-pressure lines. There is no common rail on this Cummins; that came with the 2003 engine. The ECM runs the VP44 open loop while cranking and closed loop once the engine idles, and the pump carries its own electronic control module, the PSG, on top.
The filter setup changed partway through. 1998.5-1999 trucks use a drop-down housing with 1/8 NPT test ports. 2000-2002 trucks got a top-load cartridge with a Schrader test valve on a banjo fitting. That split matters when you buy filters or plumb a gauge.
What fails and why
The factory lift pump is the weak link. It sits on the block soaking up engine heat and has to pull fuel all the way from the tank. The Turbo Diesel Register calls it marginal at best, a batch of inadequate pumps around 2001 led to a recall, and some have failed in as little as 15,000 miles. Treat it as a wear item with no fixed lifespan. Signs: low or zero pressure on a gauge, hard starting, bucking or skipping under load, a no-start after a filter change, or a system that won't prime.
When the lift pump goes, the VP44 usually follows in short order. It needs at least 5-6 psi to cool and lubricate itself, and a starved pump overheats, seizes the rotor or scuffs the distributor. Expect P0216, hard hot starts, stalling on the road and no fuel at the injectors. The PSG module on top of the pump is the other big one: its lead-free solder joints go crystalline with age and heat, and one well-known VP44 rebuilder blames it for about 90 percent of dead-pedal complaints. Codes P0251, P0253, P1688 and P1689 point at pump electronics. A long crank cold can be a cracked diaphragm inside the VP44, while a long crank only when hot points to a worn distributor, a slow starter or late fuel-relay voltage. If your fuel gauge sticks or always reads low, the tank sending unit or its barrel strainer is a known age failure on these trucks, and fixing it means dropping the tank.
Stock replacement vs. upgrade
Start with a fuel pressure gauge. Owners who know these trucks treat one as mandatory, because a failed lift pump takes the VP44 with it. A Turbo Diesel Register editor's stock 1999 showed 13-14 psi at idle and 12 psi at 2,000 rpm, but a hot-rodded truck fell to 3 psi at 30 psi of boost. The floor is 5 psi at idle and under load, and VP44 specialists recommend 15. If the truck is staying stock, a stock-style transfer pump kit is a straight replacement. If you've added power or you tow heavy, a frame-mounted system from FASS or AirDog moves the pump back toward the tank so it pushes instead of pulls, and adds filtration and air separation. Larger injectors raise lift-pump demand, so do the pump first. Replacement VP44s come in stock ratings and as performance units; match the pump to your injectors and power goal and confirm the application by VIN.
Fuel quality counts too. Ultra-low sulfur diesel cut sulfur from 500 to 15 ppm and lost lubricity along the way, and the VP44 depends on its fuel for lubrication, so a lubricity additive is cheap insurance.

Fitment notes
In these years the Cummins came only in the 2500 and 3500; no second-generation Ram 1500 had one. A 2002 2500 or 3500 is still this platform even though the 2002 1500 had moved to the new body. VP44 pumps and stock injectors are specific to engine rating and transmission, and the 245 hp High Output, a 2001-2002 option sold only with the NV5600 six-speed, uses different calibrations than the standard-output trucks. XDP lists one of its reman pumps for 2000-2002 six-speed trucks only. Filters and gauge fittings split at the 2000 model year with the housing change. Tank sump fitment depends on your tank, so check it before ordering. To pull codes, 2000-2002 trucks show them on the odometer when you cycle the key on three times in five seconds; 1998.5-1999 trucks need an OBD-II scan tool. Verify pumps and injectors by VIN.
Frequently asked questions
How do I know if my lift pump is failing?
Put a gauge on it. A gauge after the filter should hold at least 5 psi under heavy load; if it doesn't, the pump needs replacing. Bump the key to start and release it to run, and the pump should run about 25 seconds. Hard starting, bucking under load and a no-start after a filter change are the other common signs.
What causes dead pedal on a 24-valve Cummins?
Most often it's the electronic module on top of the VP44. Its lead-free solder joints go crystalline with heat cycles, so the connection comes and goes and the truck loses throttle, usually worse when hot. Codes P0251, P0253, P1688 or P1689 point at the pump electronics.
Should I check the lift pump before replacing the VP44?
Every time. A starved VP44 overheats and seizes, and pump failure follows a lift-pump failure in short order, so a new injection pump on a weak supply is at risk from the start. For a P0216 code, check lift-pump volume as well as pressure before you condemn the pump.
How often should I change the fuel filter?
Every 15,000 miles in normal service or every 7,500 in severe service. 1998.5-1999 trucks take Fleetguard FS19598 and 2000-2002 trucks take FS19855. After the change, use the 25-second prime cycles to fill the bowl; if the pump runs and the bowl stays empty, the pump has failed.
Do I need a fuel additive in a VP44 truck?
It's cheap insurance. The VP44 is lubricated by the fuel it pumps, and ultra-low sulfur diesel has less lubricity than the fuel this pump was designed around. Anti-gel only matters if the truck sees cold weather.
