On the 24-valve 5.9L Cummins, an electric Carter lift pump bolted to the side of the block, behind the fuel filter, pulls fuel from the tank and pushes it through the filter to the Bosch VP44 rotary injection pump. The VP44 uses that fuel to cool and lubricate itself, and it needs positive supply pressure to do it: at least 5-6 psi, with 15 psi recommended. The ECM runs the lift pump for about 25 seconds when you bump the key to start and release it to run, which is how you prime the system after a filter change. A stock 1999 with the factory pump measured 13-14 psi at idle, 12 psi at 2,000 rpm and 8 psi at 20 psi of boost, and a hot-rodded truck dropped to 3 psi at 30 psi of boost. The stock pump meets spec on a stock truck and runs out of margin once you add power.
What fails and why
Treat the factory pump as a wear item. Its spot on the block soaks up engine heat, and it has to pull fuel a long way from the tank instead of pushing it, which is why the 1998-2004 block-mounted pumps fail more often than the in-tank design that came later. Around 2001 a production batch of inadequate pumps led to a recall, and reported life can be as short as 15,000 miles, with no fixed failure mileage. Watch for low or zero pressure on a gauge, hard starting, bucking or skipping under load, a no-start after a filter change, or a system that will not prime. The expensive part is downstream. A starved VP44 can overheat, seize its rotor or scuff its distributor, often in short order after the lift pump goes, and it may set P0216. Before you condemn the VP44 over a P0216, check transfer-pump volume, not just pressure.
Stock replacement vs. upgrade
The simple route is an OEM-style pump on the block and a spare in the toolbox; the classic keep-it-simple advice for this truck is exactly that, with a replacement that takes about 20 minutes. Cummins' final OEM lift pump kit was 3990082, with a pigtail for all 24-valve applications, and it can be changed from above without pulling the starter. The bigger fix moves the pump. Frame-mounted systems, with FASS and AirDog the best-known examples, put the pump toward the tank so it pushes instead of pulls, and add filtration and air separation. A relocation kit that moves a stock-style pump to the frame rail near the tank is a middle path. Whichever you choose, put a fuel pressure gauge on the truck first. It is considered mandatory on a 24-valve, because a failed lift pump takes the VP44 with it. And if bigger injectors are in your plans, do the pump first, since larger injectors raise lift-pump demand.

Fitment notes
The pump on the block carried on through 2004, and 2005-up trucks moved to an in-tank pump, so order by your exact year, not by engine name alone. Fuel filter housings also changed for 2000: 1998.5-1999 trucks have a drop-down housing with 1/8 NPT test ports, while 2000-2002 trucks have a top-load cartridge and a Schrader test valve on a banjo fitting. That matters when you plumb a pressure gauge. To test a pump, bump the key to start and release it to run; the pump should run for 25 seconds, and a gauge after the filter should hold at least 5 psi under heavy load. If the pump runs but the filter bowl stays empty after a filter change, the pump has failed. Verify by VIN when in doubt.

Why does my 24-valve set P0216?
P0216 usually means the injection pump cannot reach full advance, and it often shows up before you notice any drivability problem. Check transfer-pump volume, not just pressure, before you condemn the VP44.
