Ram has used the Holset HE300VG on the 6.7L Cummins since 2013, and the 2019 engine got a revised version: taller turbine vanes, heavier-duty bearings, a forged-milled compressor wheel, laminated heat shields and up to 33 psi of boost. It sits on the passenger side, bolted between cylinders 4 and 5. An electronic actuator moves a ring of vanes in the turbine housing to change how exhaust hits the wheel. That same mechanism closes down to make the factory exhaust brake, which is why turbo health and braking performance go together on this truck. For 2025 Ram moved to a Gen II HE300VG, so check the year range before you order.
What fails and why
The big one is what owners call Stuck Turbo Syndrome. Soot and oil coke up the vane ring and pivots until the vanes bind. The exhaust brake weakens first. Then come low or erratic boost, black smoke, high EGTs and limp mode, with codes such as P003A, P2262, P226C, P00AF, P0299 for underboost and P0234 for overboost. It commonly shows up between 50,000 and 150,000 miles, and sooner on trucks that make short trips, idle a lot or run extended oil drains. Bearing wear and oil seal failure are less common, but they condemn the cartridge; look for a siren whine, blue or gray smoke and shaft play. One diesel shop reports slow spool and whine on turbos past about 120,000 miles.
Keep it alive, or replace it
Prevention is cheap. Stick to factory oil intervals instead of stretching them, warm the engine fully before you load it, let it idle down after a hard pull, keep a clean OEM-spec air filter in it and use the exhaust brake, which cycles the vanes and is the best defense against sticking. The 2009-and-later turbine housing has an access port for vane cleaning, and on a high-mileage unit a cleaning can buy 12 to 24 months. When the turbo is done, a direct-replacement HE300VG keeps every factory function, including the exhaust brake, EGR and DPF/SCR compatibility, and runs on factory tuning. That is the path for a truck that works for a living.
Performance turbos are a different decision. Our research has not turned up a CARB Executive Order number for a compound kit or a larger drop-in turbo on the 2019-2024 truck, so read each listing for what it requires and confirm it is legal for your truck and state before you buy.

Fitment notes
Know which engine you have before you order a turbo or actuator. The high-output engine is a 3500-pickup-only option; sales code ETM marks a high-output truck and ETL a standard-output one. Check year ranges closely: the Fleece Cheetah is listed for 2019-2022, and several Fleece service parts, including the turbine shaft speed sensor, stop at 2022 in their titles. Chassis cab trucks share the engine family but run their own calibration, so give us the VIN. And none of these turbos are listed for 2025-and-later trucks, which use the Gen II HE300VG.
Frequently asked questions
My exhaust brake feels weaker than it used to. Is the turbo going bad?
It can be the first sign of sticking vanes. On this engine the VGT doubles as the exhaust brake, and soot and oil coking on the vane ring shows up as a weak brake before low boost, black smoke or limp mode. Get it scanned for codes like P003A or P2262 before you buy parts.
Can a sticking turbo be cleaned instead of replaced?
Sometimes. The turbine housing on 2009-and-later units has an access port for vane cleaning, and on a high-mileage turbo a cleaning can buy 12 to 24 months. If the bearings are worn, with a whine, blue or gray smoke or shaft play, cleaning will not fix it.
Do I need a tuner with a replacement turbo?
Not with a direct replacement. A direct-fit HE300VG keeps the exhaust brake and all emissions functions and runs on factory tuning. Performance turbos are a separate decision, so read each listing for what it requires.
How do I tell if my truck has the high-output engine?
The high-output 6.7L was only offered in the 3500 pickup, behind the Aisin AS69RC six-speed. Ram's sales code ETM marks a high-output truck and ETL a standard-output one. Send us the VIN and we will check it before you order a turbo or actuator.
