A first-gen Ram's suspension is simple and mechanical. The working parts list is leaf springs, shackles, overload springs, shocks, ball joints and sway-bar bushings. Every one of these trucks has leaf springs in the rear. 4WD W-series trucks have leaf springs up front too, while 2WD D-series trucks use coil-spring independent front suspension. Air bags and traction bars are the common add-ons for trucks that tow or make more than the factory 160 hp and 400 lb-ft from the 5.9L Cummins.
What fails and why
Rust at the spring perches, the cab mounts and the frame behind the fuel tank is the primary first-gen concern, and it hits rust-belt trucks at any mileage. Ball joints and tie-rod ends wear with miles and show up as clunks, wander and uneven tire wear. Owners who add power report axle wrap and wheel hop from the leaf-spring rear, sometimes with driveline shudder on launch.
Stock replacement vs. upgrade
For a stock refresh, Bilstein shocks and new sway-bar bushings are documented OEM-style parts on first-gens. For towing and hauling, rear air-bag helper springs with a compressor are the documented upgrade. For a truck with more power, traction bars control the leaf-spring rear. Four-link and air-ride rear conversions exist, but they live on custom show builds.

Fitment notes
2WD and 4WD first-gens take completely different front suspension parts. Mid-1991-and-later 250 Cummins trucks ride on 350 frames, so bracket and spring fitment may follow the frame instead of the badge. The Bilstein 5100 listing is labeled for the front of a 2014-2026 Ram 2500 6.7L 4WD, which is a later truck, not a first-gen. Universal traction bars and air spring hardware have to be matched to your truck, so verify by VIN or call.
Frequently asked questions
Do air helper springs replace my leaf springs?
No. Helper springs work alongside the leaf springs, which is how first-gen owners who tow and haul run them. If your truck has the optional factory overload leaves, mention that when you call so we can check bracket and kit fitment.
Are traction bars only for high-horsepower first-gens?
They show up mostly on high-output builds, because that is where owners report axle wrap and wheel hop from the leaf-spring rear. If your truck isn't hopping or shuddering on launch, you may not need them yet.
Which shock fits a 1989-1993 Ram?
Bilstein shocks are a documented OEM-style refresh on first-gens, but the Bilstein listing in this collection is labeled for the front of a 2014-2026 Ram 2500 6.7L 4WD. Give us your drivetrain and ride height and we will help you find a first-gen part number.
