Shock choice on a first-gen starts with how the truck is sprung. 4WD W250 and W350 trucks ride on leaf springs at both ends with a solid front axle. 2WD D250 and D350 trucks use coil-spring independent front suspension with leaf springs in the rear. Those two front ends take completely different parts, shocks included, so drivetrain comes first. Shocks sit on the first-gen component list alongside leaf springs, shackles, overload springs, ball joints and sway-bar bushings, and on a stock-height truck, Bilstein shocks with new sway-bar bushings are a documented OEM-style refresh. The 5.9L Cummins hub covers the rest of the platform.
Stock height vs. lifted
Ride height decides shock length. At factory height, an OEM-style shock is what the truck was built around. Lifted first-gens are a different case: the documented 4-inch leaf-spring lifts on W250 and W350 trucks ran their own shocks chosen for the lift, so if your truck has been raised, start from the lift height, not the factory part number.
What we carry
The Bilstein listing here is a B8 5100 Series shock absorber, and its title lists the front of a 2014-2026 Ram 2500 6.7L Cummins 4WD. That is a later truck's fitment, not a 1989-1993 fitment.
Fitment notes
Give us drivetrain (D or W), 250 or 350, single or dual rear wheels, and any lift height, and verify by VIN. Mid-1991-and-later 250 Cummins trucks sit on 350 frames, so some suspension parts follow the frame rather than the badge.

Plan on it. The documented 4-inch leaf-spring lifts on W250 and W350 trucks ran shocks chosen for the lift, because the new ride height changes the length the shock has to cover.
