The Cummins 5.9L in these trucks is a cast-iron inline six with a forged-steel crank on seven main bearings, overhead valves worked by a solid-lifter camshaft, and four valves per cylinder. The cam is gear-driven, which is why a cam gear and a timing gear case gasket show up on this page. Valve bridges tie each pair of intake or exhaust valves together under a single rocker. For the common-rail engine, Cummins stiffened the block with sculpted side walls, stiffening rails and a plate under the main caps to carry the higher cylinder pressures. The 2003-2005 engines also wear a black plastic crankcase-breather cover on top of an aluminum rocker housing.
What to know before you tear in
Heat is the internal enemy on this engine: after repeated high exhaust-temperature excursions, a dropped valve seat is usually the first thing to go. Top-end work has its own traps. Pulling an injector means taking the exhaust rockers off, so keep rockers and injection lines in cylinder order; the line nuts are etched with sequential part numbers. Rocker bolts torque to 27 ft-lb and the valve cover to 18 ft-lb. Valve lash is a 150,000-mile job, reset cold to 0.010 in intake and 0.020 in exhaust. And if you are replacing freeze plugs, the #53 block cracking problem belongs to 1999-2001 blocks, not yours.
Stock replacement vs. upgrade
For a stock rebuild or a leak fix, the factory-style parts are here: Cummins genuine front crankshaft seals with or without a wear sleeve, rear main seals including one with a repair sleeve, upper and lower engine gasket kits, the cam gear, the oil pickup tube and the crankcase breather filter. MAHLE adds rear main seals, a timing gear case gasket, a timing cover seal, a lower gasket set and head sets, and Dorman covers the dipstick, dipstick tube and steel cup expansion plugs. When boost and rpm go up, billet rocker trunnions, valve bridges and upgraded valve springs are the common valvetrain upgrades, with head studs once fueling rises. XDP offers HD rocker arm assemblies, an HD trunnion set with polishing tool, a billet valve bridge set, high-boost and performance valve spring sets, street performance pushrods and an X220 head stud kit, plus HD bolt-on freeze plugs for the side and rear of the block.

Fitment notes
This page is for the Dodge Ram 2500 and 3500 with the 5.9L Cummins, not the Ram 1500. Year ranges in the titles vary a lot, and they mean what they say. Valvetrain parts listed 1998.5-2024 cover the 5.9L and 6.7L years shown, including yours. Gasket kits and head sets listed 2003-2007 fit the common-rail 5.9L. The breather filter is listed 2003-2005 because the 2006-2007 rocker housing went to a molded plastic cover with an integral breather. Vendors also split 2003-2004 from 2004.5-2007 on several parts, so confirm the build date and verify by VIN when in doubt.
Frequently asked questions
Do I need to worry about the #53 block on a 2003-2004?
No. The #53 casting cracks below the passenger-side freeze plugs on 1999-2001 24-valve blocks with thin water-jacket walls. Your 2003-2004 common-rail block falls outside that range, so freeze plugs here are ordinary service parts.
Which valvetrain parts make sense when I add boost?
The common upgrades for raised rpm and boost are billet rocker trunnions, billet valve bridges and stronger valve springs, with pushrods to match. On this page that means the XDP HD trunnion set, billet bridge set, high-boost or performance valve springs and street performance pushrods. Plan head studs at the same time if fueling is going up too.
How tight do the rocker bolts go?
Rocker bolts torque to 27 ft-lb on the common-rail 5.9L. If you are pulling injectors, the exhaust rockers come off, so keep rockers and injection lines in cylinder order. Valve cover bolts go back at 18 ft-lb.
