The 2019 redesign gave the frame a fully boxed rear section and an upgraded rear-axle crossmember with fifth-wheel and gooseneck provisions. That year the top fifth-wheel and gooseneck rating was 35,100 lb, on a 3500 regular cab 4x2 dually with the high-output engine and 4.10 gears. For 2021, Ram upgraded the rear crossmember again, added its own in-house hitch, and raised the gooseneck rating to 37,100 lb on the 3500 dually with the high-output engine. Trucks with the prep also get a 7-pin plug in the bed.
Factory casting or aftermarket hitch?
If your truck has the factory center casting, a gooseneck ball and bolt-plate accessory locks into it, and on 2021 and later trucks those pieces came with the prep. If your truck was built without the prep group, Mopar prep kits add the pucks. Before you hook up
Two recalls on 2019-2022 trucks touch the bed and tailgate. Recall 20V-207 covered an accessory bed step that could fail, and 22V-904 covered tailgate strikers that could let the tailgate open. Neither is a hitch defect, but gooseneck hookup means climbing in and out of the bed to set the ball and chains, so have the recall status checked by VIN on any truck you bought used.

Fitment notes
Look in the bed for four pucks and a 7-pin plug; that's the sign of the factory prep group. Check the model years on the hitch listing against your truck, since the bed floor changed for 2021. Remember that the 37,100-lb rating belongs to a 3500 dually with the high-output engine; a 2500 is rated far lower, at 15,520 to 19,780 lb in Ram's 2019 chart. Verify by VIN, or call and we'll help. Does my truck already have a gooseneck setup?
Look in the bed. A truck with the factory prep has four pucks, a center casting for the gooseneck ball and a 7-pin plug in the bed. On 2021 and later trucks with the prep, those are integrated into the box floor. If you see none of that, the truck was most likely built without the prep.
What's the highest gooseneck rating on this generation?
37,100 lb, starting in 2021, on a 3500 dually with the high-output engine. In 2019 the top fifth-wheel and gooseneck figure was 35,100 lb, with a 43,000-lb GCWR, on a 3500 regular cab 4x2 dually with the high-output engine and 4.10 gears. A 2500 or a single-rear-wheel 3500 is rated well below that, so check your build.
Can I add gooseneck capability to a truck built without the prep?
