The 24-valve 5.9L Cummins uses an electric grid sandwiched between the intake horn and the intake manifold as its cold-start aid. Two relays power it. The computer watches both relay circuits, logging P0380 for relay 1 and P0382 for relay 2, and it checks for a temperature rise from the heaters. The wait-to-start lamp is tied to the same system and has its own code, P0381.

What fails and why

Relay age is the usual story. Faults in the relay circuits set P0380 or P0382. P1291 reads as no temperature rise seen from the intake air heaters, which flags a dead grid. On the truck, that looks like hard cold starts, white smoke when cold and odd wait-to-start lamp behavior. Stock replacement vs. upgrade
