The service list for a first-gen 5.9L Cummins cooling system is short and familiar: a 180°F thermostat and its gasket, the water pump and the O-ring that seals it to the block, upper and lower radiator hoses, heater core lines and the block's expansion plugs. The engine oil cooler and its gaskets share this circuit on 1989-2002 engines, so any coolant job touches more than the radiator.
What wears out
The thermostat is a wear item. As it ages it sticks open, which gives you slow warm-up and weak heat, or sticks closed, which overheats the engine. The water pump starts weeping at high mileage; watch for coolant at the weep hole, a coolant smell or a low overflow bottle. On 1989-1998 non-storm blocks the freeze plugs rust through and leak from the side of the block or from the rear behind the flywheel. Hoses and gaskets are on the routine service list, and the sensible time to replace them is while the system is already drained.
Stock replacement vs. upgrade
For thermostats, the genuine Cummins 5292738 holds the factory 180°F rating. Mishimoto's high- and low-temperature thermostats give you a different opening point; check each rating on the product page before you move away from stock. For hoses, Dayco's curved and flex hoses are year-matched replacements, and Mishimoto's silicone radiator hose kit is the upgrade route for 1991-1993 trucks. On the pump side, the XDP XTRA Cool water pump is listed for 1989-2012 5.9L engines.

Fitment notes
Hoses are where first-gen orders go wrong. The intercooler arrived mid-1991, and the hose listings follow that split, but vendors don't always label the years the same way. Look for the air-to-air intercooler in front of the radiator and order to match what you see. The thermostats here are listed for 1989-1993. Expansion plug fixes are specified for 1989-1998 non-storm blocks, and the 1989-1993 block falls in that range.

Which radiator hoses fit my first-gen Cummins?
It depends on which side of the mid-1991 intercooler change your truck falls. Dayco lists separate upper and lower hoses for 1989-1991 and 1991.5-1993 trucks, and one curved hose for the full 1989-1993 run. Check for the factory intercooler before you order.
Should I run a high- or low-temperature thermostat instead of the stock 180°F?
The factory rating is 180°F, and that's the baseline for a stock truck. Mishimoto's high- and low-temperature versions open at different points, which are listed on each product page. If you're unsure which suits how you use the truck, call the shop before you buy.
Do I need a new seal when I replace the water pump?
Yes. The pump seals to the block with an O-ring (Cummins 3906698), and it should be new whenever a pump goes on. MAHLE's K31420 water pump seal is listed for 1989-2017 5.9L and 6.7L Cummins; compare it with the seal on your old pump.
